A hierarchical network meta-analyses of randomized control trials, comparing structured exercise interventions with non-exercise control or alternative exercise modalities in middle aged and older adults, and its impact on Bone mineral density (BMD), concluded that exercise is effective in reducing age-related decline in Bone-Mineral density.

The authors advocate cautious support for the integration of dose-specific, modality tailored exercise prescriptions, into bone health guidance.

Clinically Meaningful BMD gains might be expected with 2-3 hours of brisk walking, equivalent to 600 METS-min/week, suggesting that extremely high intensity regimens may not be necessary.

Mixed Aerobic and mind body exercises might improve fracture risk.

Learning points

Brisk walking or jogging and combined aerobic-resistance exercises probably provide the greatest BMD benefits (Low to moderate certainty quality evidence).

Minimum of about 600 METs-min/week which achievable with about 2-3 hours of brisk walking or jogging, is generally required for clinically meaningful benefits.

As, Age and increased adiposity may blunt skeletal sensitivity to mechanical stimuli and older adults with overweight or obesity may require higher doses to achieve similar effects, Exercise prescriptions need to be tailored and periodised.
